CHRISTAMORE HOUSE FAMILY AND COMMUNITY CENTER

Position: Bi-Lingual Case Manager (Full-Time)

Reports To: Director of Family Services

The Bi-Lingual Case Manager will be responsible for conducting case management services to Haughville’s residents with the primary objective to enhance awareness, develop improved access to services and engage program participation available at the Christamore House and beyond.

Responsibilities

  • Provide case management and specialized supportive outreach services to prospective clients residing in the Haughville area of Indianapolis, encouraging participation in agency’s programs.
  • At least 15% of the work week will be spent out of the office within the community for outreach.
  • Provide Spanish/English language translation services.
  • Ensure ongoing growth in skills levels, promote economic growth and experience working with issues impacting and affecting individuals and families living within the Haughville area (i.e workshops, etc.)
  • Develop and maintain high quality case notes/files on each individual and family seeking services.
  • Provide assessment of individuals and families needs; translate for individuals/families as needed.
  • Promote and engage prospective new clients by increasing the number of resident participation in all organizations’ programs.
  • Maintain monthly contact with clients and families enrolled in the Family Strengthening Partnership and other case management programs.
  • Keep track of total numbers served for accurate monthly reporting; Must be outcome-based driven-measures focused.
  • Serve as a resource for individuals seeking services including problem solving skills, ideas, available community resources and dealing with problems and crisis arising with program individuals and families.
  • Remain in consistent contact with partnering organizations.
  • Attend scheduled departmental and agency meetings.
  • Maintain in close communication with immediate supervisor, Director of Operations and Executive Director, as well as funders.
  • Other duties as requested.

 

Requirements

  • Bachelor’s degree in Sociology, Human Services or related field is required.
  • Must have 2+ years of experience in providing case management and outreach services.
  • Experience with donor expectations and grant writing is highly desirable.
  • Must be highly fluent in both English and Spanish.
  • Inhabit strong presentation and writing skills.
  • Must have exceptional oral, written and interpersonal communication skills.
  • Must be well organized, detail oriented and have the ability to maintain confidentiality.
  • Ability to positively work with diverse client populations, community organizations and staff members.
  • Current Indiana Drivers’ license and valid insurance to deliver outreach services.
  • Considerable knowledge of community resources to include housing resources, legal aid services and employment training programs.
  • Ability to operate and navigate through Microsoft Office software effectively.
  • Ability to work in crisis situations and remain calm.
  • Ability to keep the confidentiality nature of all client files and all agency situations.
  • Have an established network of various multi-cultural resources.
